Podophyllotoxin is a locally acting cytostatic agent and can be applied by the patient himself as a 0.5–2% tincture. Within 1–2 days, an erosion develops in which the condylomata melts. After 7 days, the defect is re-epithelialized with normal skin. Trichloroacetic acid is equivalent as a cytostatic agent and is widely used in Africa, but is not used in Germany.

Imiquimod is a topical immunomodulator. It acts as an agonist for the toll-like receptor 7, which stimulates macrophages and keratinocytes to secrete pro-inflammatory cytokines. Imiqimod is available as a 5% cream (Aldara). Treatment is carried out by the patient himself until the papillomas have healed, which may take several weeks. Erythema, edema, burning sensation, and local pain can all occur as side effects, which must be pointed out to patients so that they do not stop treatment too soon. The success rate is around 40% and the relapse rate around 10%.

Polyphenon E is an extract from green tea, the main active ingredient of which is a catechin, which acts on several cellular signaling pathways. Local treatment is also possible with this (Veregen), with similar success and recurrence rates.

A high rate of recurrence of around 25% is to be expected with all treatment measures. Relapses must be treated again, including with alternating procedures, until the disease is gone (Stanley 2012).

Healing is made more difficult by the phenomenon of latency. The HPV DNA can persist in the epithelial cells for years. A renewed outbreak of the disease is unlikely in immunocompetent people, but reactivation can occur if immunodeficiency occurs. In all extensive condyloma disease, the possibility of T-cell dysfunction should be considered and appropriate investigations should be initiated. Extensive and difficult to influence papilloma diseases are found in HIV-infected people in particular.
